Accident caused by faulty machinery

Mr W of Yorkshire was injured whilst working as a factory worker in a large industrial factory using old heavy machines. He had his accident just before Christmas.

His managers knew his machine was dangerous and faulty but insisted that he use it. The emergency stop button on the machine didn’t work and there was no safety guard. Mr W needed to keep his job so did what he was told.
